RSPCA Report Cruelty Form Re-design

The Problem
The report abuse forms are inconsistent as different states have different forms and often overwhelming for users as they get confused by different forms.
This got us thinking, how can we enhance the RSPCA website's reporting platform to swiftly empower users, making it more user-friendly and efficient for advocating animal rights and reporting animal abuse?
The Solution
We aim to simplify reporting of animal cruelty through a unified platform and national contact, prioritizing user experience and cohesive design across RSPCA sites, while emphasizing the "Report Animal Cruelty" feature for impactful action.
From the user interview data, we were able to draw an affinity diagram, which lead us to our user persona and her story of reporting animal cruelty

We have taken form design inspiration from many resources with an extensive research. We focused on better readability, being straightforward in nature and simplicity.

We have used breadcrumb navigation as progress indicator
With an extensive user test, we were able to make the logical rearrangement of the steps of the form. We re-arranged the steps of the form as it suited the users better.
